Sole Shine Barefoot Massage Therapy Membership

With no long term contract and the option to roll over sessions, you receive Sole Shine
member perks, including one 60 minute therapeutic massage at the membership rate every month, automatically charged to your account on the ____ day of every month.
Unused 1 hour member massages do not expire while you maintain an active membership.
Sessions carry over from month to month and will be accumulated to your account.
We require a credit or debit card to be left on file.
Every 6 months, you will be asked to confirm that the credit and billing info is up to date. There is an option for writing a check. See below for more info.
Additional Time: We believe massage therapy is our art form and while this is also your individualized session, we believe our creative inspiration is best when there is no restriction based on therapeutic add on treatments (aromatherapy, hot pillows etc). We do not charge extra for treatments but instead, additional time as you reserve in advance. Our additional treatments are included in the price and based on customer and therapist in combination with specifically indicated needs.
***Each membership will run at the base price of $70 and as you schedule your appointment, if you wish to receive a longer session, you will be asked to pay the difference at the time of your session. To add more time, schedule your session and at the time of service you will be charged accordingly.
* 15 mins = $15 *30 Mins = $30 *2 hour sessions will always be $150.
Members Only Perks
- Extended Hours: Available outside of our posted hours for our members only!
- Additional Sessions: Pay $70 for 60 minute sessions outside of automatic monthly massage.
- Share: If the event arises that massage sessions are accumulated on a member account, you are welcome to share 1 massage per month @ the transfer fee of only $10 per session.
- Give a Gift: Purchase up to 2 gift certificates per month @ member rate of $70
- Retail Discount: Save 10% on all retail products.
- Referral Program earn $5 credit to your account every time you send in a referral.* This credit may be applied towards additional massages, retail products, select classes/events and gift certificates only. This credit can not be applied to monthly member payment. *Credit is applied once the referral books and completes their scheduled appointment.

___ There is a one time $10 setup fee
___ if you would like to add the same amount for gratuity for each session, we can include that in your recurring payment. This is a convenience for you and not required. Simply initial here and include amount: $_____.____
___ Check Option: You are able to write a check for your membership. However, the check must be mailed to Sole Shine Barefoot Massage Therapy on or before the first of every month. Using this function waives your ability to choose the date your membership is charged. If you are late on your payment, your membership will automatically go into freeze. For more information on a frozen membership, read below.
___ Splitting time between sessions is not allowed.
___ You are receiving a discount & perks by pre-paying for therapeutic massage sessions.
___ Prices are subject to change at any time but plenty of notice will be given.
___ The prepaid massages are non-refundable. If the purchaser, for any reason, is not able to use the prepaid massages, an agreement will be made between Sara Newberry, LMT and purchaser regarding the unused discounted massage sessions.
___ If at any time, you need to freeze your membership, there will be a $10 fee required to put your member account on hold for 3 months. At this time, monthly payments will stop being drawn from the card on file. Members with a held account can redeem accumulated service during a freeze time but cannot utilize any additional member benefits. Your account will automatically resume after the 3 month freeze period ends. If you choose not to resume your account please refer to the cancellation policy.
___ Your membership may be canceled at any time;
We require a written notice in the form of a hand-written letter or e-mail
30 days before you wish to cancel your membership.
After required notice is given, one last pre-paid massage will be charged to your account.
___ Upon termination, accumulated member services expire & are non-refundable 60 days after a membership is canceled.
For scheduled appointments, a 24 hour cancellation notice is required.
___ Our Cancellation and No Call No Show policy will be strongly enforced.
It is understood that last minute cancellations are a part of our changing lives however; we ask that you use courtesy in managing your scheduled appointments with us. Time is our (& yours) most valuable asset. When you miss an appointment, you not only affect your therapists’ schedule, but you do not allow for another person to receive their valued hour of relaxation.
___ If you miss an appointment without a 24 hour notice you will be required to pay a fee of $40 for the missed session or forfeit a prepaid massage.
___ In the event an appointment is missed, you will receive an invoice for the missed cancellation fee. At that time, you can make a payment or contact Sara Newberry, LMT to choose to forfeit a prepaid massage.
Sole Shine Barefoot Massage Therapy reserves the right to assume you have made the appropriate measures to ensure your availability to receive your scheduled session. It is with generosity that we provide an email and, in some cases a text to remind you of your appointment.
